Manage your conditions by exploring exercises for better knee health and discover strategies to reduce risk and manage metabolic conditions.
Manage Metabolic Health
A 6-week foundational Active Health programme with pre- and post-assessments (follow-up assessment in 3rd month), designed to help individuals manage metabolic health and reduce chronic disease risks through structured movement strategies and health coaching whilst learning evidence-based approaches to improve overall wellbeing. Suitable for individuals aged 18 and above who meet the eligibility criteria may enroll. Review the full criteria at go.gov.sg/managemetabolic before signing up.
Suitable for the individuals aged 18 and above who meet the following eligibility criteria:
- With well-controlled type 2 diabetes mellitus (T2DM) and no hypoglycaemic episodes in the past three months
- With BMI between 23–32.5 kg/m² and/or waist circumference >90cm for men and >80cm for women
- With well-controlled hypertension, maintaining resting systolic and diastolic blood pressure readings of <160 and <100 mmHg respectively
- With elevated triglyceride levels and low HDL cholesterol levels
